1. Completion and submission of all application forms by parents/guardians.

2. Completion of medical form by child's physician with up to date shot records.

3. Copy of Medicaid or health insurance card.

4. Orientation visit by child and conference with parent(s) of the child.  At this conference,     

        school policies will be explained to be certain that there is a clear understanding of what is

        required for each child, what is expected of parents, and what services the school offers.

5. Payment of registration fee, first two weeks, or first month's fee in advance.
